個人資料蒐集處理利用告知事項暨同意書
歡迎您使用國立歷史博物館「國立歷史博物館-數位文創加值網」各項服務並申請加入會員的行列，為保障您的使用權益，請於註冊成為「會員」之前，詳細閱覽以下內容。
國立歷史博物館 (以下稱本館)為執行「數位跨域文創資源加值網站」之服務，需蒐集、處理及利用您的個人資料，為保障您的正當權益，請務必詳閱以下所述事項：
一、機關或單位名稱：國立歷史博物館
二、蒐集之目的：為執行數位跨域文創資源加值網站之服務。
三、個人資料之類別：(C001)辨識個人者/(C002) 辨識財務者/(C003)政府資料中之辨識者/(C101) 資料主體之商業活動/(C102) 約定或契約/(C103) 與營業有關之執照
四、個人資料利用之期間、地區、對象及方式：除涉及國際業務或活動外，您的個人資料僅供本館於中華民國領域、在前述蒐集目的之必要範圍內，由本館自行利用，或依法令規定提供予本館目的事業主管機關、其他第三方，本館將透過數位檔案或實體紙本形式蒐集、處理及利用您的個人資料，以合理方式（包含書面、電子等）利用至蒐集目的全部消失為止。
五、當事人應以書面來函請求行使個人資料保護法第三條當事人權利規定，詳細敘明所欲行使之權利種類、內容以及連絡資訊；若未具備上述要件者，本館將通知您補件，於完成補件後始完成請求程序，並開始起算辦理期間，以避免因資料不備而損害您的權利。
前項所稱當事人權利包含：查詢或閱覽、製給複製本、補充或更正、停止蒐集處理或利用，必要時亦可請求刪除，惟屬本館依法執行職務所必須保留者、或於特定目的全部消失前，得不依台端請求為之。您在向本館請求補充或更正時，應為為適當之釋明。且您在向本館查詢、請求閱覽或請求製給副本，本館得酌收必要之成本費用。
六、您有權選擇是否提供個人資料予本館蒐集、處理利用。若您不提供個人資料或提供後向本館請求刪除時，本館將無法受理或將提前終止與您之業務往來或其他任何服務。

Notification and Consent about Gathering, Processing and Utilization of Personal Data
Welcome to use all services on Digital Cultural and Creative Website of National Museum of History and register as a member. Please read carefully the following contents before your registration to ensure your usage rights.
National Museum of History will gather, process and utilize your personal data to execute the services on Digital Cross-domain Cultural and Creative Resources Website. Please read the following contents to guarantee your legitimate rights:
1. Name of organ or unit: National Museum of History
2. Objective of gathering: to execute the services on Digital Cross-domain Cultural and Creative Resources Website.
3. Category of personal data: (C001) individual recognition/(C002)financer recognition/(C003) recognition of governmental data /(C101) business activity of data body /(C102) agreements or contract /(C103) licenses related to business
4. Period, region, object and means for personal data utilization: except international business or activity, your personal data is only used in Taiwan by this museum or provided to institutional competent organ or the third party according to laws and decrees within the necessary range. This museum will gather, process and utilize your personal data through digital archive or practical paper edition and use the data in appropriate ways (including written form or electronic form) till all the gathering targets disappear.
5. Parties concerned shall request to execute the rights of the parties concerned in Article 3 of Personal Data Protection Act in written form, and specify the categories, contents and contact information of rights to be executed. This museum will notify you for supplement in case of insufficiencies. Then the request procedures are completed after insufficiencies supplemented and transaction period is calculated to prevent the insufficient data from damaging your rights.
Rights of parties concerned referred above include inquiry or read, copies making, supplement or modifications, gathering or utilization stopping or deletion upon request, except those can't be deleted for this museum to execute its duties or specific objective disappears. You must give appropriate illustrations when request for supplement or change. Meanwhile, this museum will collect necessary cost from you for your read or copies making.
6. You have the right to decide whether to provide personal data for gathering, processing and utilization of this museum. If you fail to provide personal data or request this museum to delete the data, this museum will not accept or terminate any business or services with you in advance.
